Care Navigators will work with you to better understand your health care needs and find ways to solve your problems. Our Care Navigators will help you create a plan of care that is based on your needs and health care goals. Care Navigators can help you find providers, schedule appointments, arrange transportation, and more.
Working with a care navigator means you have someone by your side, helping with communicate with your relatives, providers, and community supports.
WHO IS ELIGIBLE FOR CARE MANAGEMENT?
Active Medicaid
Individual must have active Medicaid coverage.
Residency Requirement
Must live in Erie, Niagara, or Wyoming County.
Medical Conditions
- Two or more chronic conditions
- Living with HIV/AIDS OR a significant mental illness.
